Workshop consent
Ruby Raut from WUKA is running a period health workshop with an interactive session at Inspire Girls. Ruby and Alfie from WUKA would also like to hear girls' experiences, so we can improve our courses and develop new ones for girls and coaches. Please read this together, then each choose one option.
What happens on the day
- Ruby and Alfie film the workshop and chats (video and sound) on WUKA cameras. A club adult is always nearby.
- Every girl can join in fully, whichever option she chooses.
- The raw recording is only seen by Ruby, Alfie and David Slocombe, and is deleted within 90 days. Only what your option allows is kept.
- Never your name. Never in adverts. The club, Inspire Girls, may be named.
- You can skip any question, stop at any time, and you don't have to talk about your own health.
- If you tell us something that worries us about your safety, we'll tell the club's welfare officer.

- Your signed form is ready as a PDF.
- In the menu that opened, choose Mail. The PDF is already attached.
- Paste dpo@wuka.co.uk into To, then send. WUKA will reply to confirm.
If no menu opened, the PDF has downloaded: attach it to an email from Files, then Downloads. Or copy your answers and paste them into the email. Your typed names count as your signatures.
Looking after your information
Raw recordings are deleted within 90 days. Words and clips your option allows are kept while we use them. This form is kept while we use anything of hers, so we can show we had permission, then destroyed.
Changing your mind or any concern: contact David Slocombe, Data Protection Manager, at dpo@wuka.co.uk first. You can move to an option nearer A, or withdraw, at any time. We'll remove WUKA posts she's in and keep her out of new material, though we can't recall anything already printed. You can also ask to see or delete what we hold. If you're still not happy after talking to us, you can contact the Information Commissioner's Office (ico.org.uk).
